Survey: help improve family centered care

When families and health care providers work together, this is called family-centered care . Family Voices is a national organization that works to achieve family-centered care for all children and youth with special health care needs and/or disabilities. We are doing research to develop a way to measure whether health care providers are delivering family-centered care.You can help by taking an online version of a family-centered care assessment tool to test how well it works. It will take 15 - 20 minutes to complete and will: Ask some questions about you and your child. (Nothing that will identify you or your child personally.) Ask about how your health care provider works with you and your child. (Your provider also won't be identified.) NOTE: Your participation is completely voluntar

New Medicaid Transportation fact sheets available

Family Voices Indiana gets many requests from families and professionals about transportation services for individuals with special health care needs. So, we created a series of fact sheets to outline the process under different programs: medical transportation under Hoosier Healthwise, Medicaid Disability/Traditional Medicaid; and transportation services under the waiver. The latter is still being developed by DDRS, so be sure to check the fact sheet for the latest update to information.
