COVID-19 individual and family survey

The Bureau of Developmental Disabilities Services values the collaboration and input from the individuals and families who use our services. We want to hear from you on your experiences and opinions on accessing and utilizing our services during the COVID-19 public health emergency. We have developed two surveys to assist us in responding to trends and enhancing the services you receive.

• Individuals with disabilities and their families who are receiving through the Family Support Waiver or the Community Integration and Habilitation Waiver can access their survey by clicking here. Individuals who are receiving these services may live in the family home, in a home by themselves or in a home with up to 4 individuals in a household.
• Individuals with disabilities and their families who are receiving services through Supervised Group Living (group homes) can access their survey by clicking here. Individuals who are receiving these services may live in a home with up to eight people.
