Take the Americans with Disabilities Act at 30 Survey for Hoosiers

You are invited to complete this survey about the implementation of the Americans with Disabilities Act (ADA) in Indiana. The purpose of this survey is to ask Hoosiers with disabilities, family members, people in the disability field, and interested others to share their thoughts about the ADA as we commemorate the 30th anniversary of this civil rights law for people with disabilities. The results will help us better understand how Indiana communities are doing in complying with the ADA.

 

The survey should take you about 10-15 minutes to complete. Your answers will be anonymous. Your participation in this survey is completely voluntary, and you may stop answering at any time.
