FUN Activities Survey (Potential Library Program)

A teenager in Carmel, Indiana has created a venture called Friendship Unlimited Network (FUN), which is for children with special needs. The programs provide immersive, hands-on, and sensory-friendly activities because all children deserve to have fun experiences. They are planning to host events at local libraries for younger children, with story time followed by activities related to the content read. However, in order to best serve children with special needs, they need to understand which activities children enjoy and which activities should be left out of the program. 

This form was created to survey parents about what children want.  Once enough information is received, Carmel Clay Public Library will be contacted about starting the program, and all children and parents involved will be invited.
