Consumer information about group home conversions

As an individual residing in an Indiana group home through the Supervised Group Living (SGL)
Program, I am writing to you to share some upcoming changes and opportunities that will be
happening throughout 2013, 2014, and 2015.


Indiana recently received funding from the federal government to transition individuals from
group home funding to home and community based waiver funding. For many of you, this is
something you have waited for a long time. For others, this is an opportunity to receive
additional services and options that you may have not had in the past.


Individuals who transition from group home will receive a Community Integration and
Habilitation Medicaid Waiver (CIH). With the CIH waiver, individuals and families will have
greater choice and control over their services, supports, and living situation. They may also
select providers for a variety of day programming, behavior management, and residential
services. Individuals may be excited to learn that they have the opportunity to receive new
services that may not have been available in their group home such as music therapy,
vocational workshop, or recreational therapy.


Transitions will occur by group home over the next couple years. You will be notified by your
group home provider when your home is going to transition to waiver. The provider will then
have an informational meeting for guardians and individuals to explain the transition process
and answer any questions you may have. You can also see our frequently asked questions on

also continue to have Powerful Parent meetings around the state to discuss group home
conversions and transitions. The first meeting for 2013 was held on March 13, 2013 in Fort
