Indiana State Department of Health Priorities
Please help the Maternal and Child Health and Children's Special Health Care Services at the Indiana State Department of Health (ISDH) determine what should be our highest priorities for the next several years by completing the attached survey. It consists of a list of topics which you’ll divide into different groups based on the instructions below. If you’ve already completed this survey once, please do not complete it a second time. Thank you very much for your time.
First, look at the list of topics. Each topic is a problem. You’ll notice some are more general than others. Also there is space at the bottom of the list for you to add anything you feel is important that isn’t listed.
Now pick the 3 topics you think should be the Maternal and Child Health and Children’s Special Health Care Services’ first problems to work on. Put the number 1 in the space to the left of each of those topics.
Now pick 3 more out of the remaining topics, but this time put the number 2 beside each of them.
Next pick the top 4 of the topics that are left. Put the number 3 beside each of those.
Next pick 5 more topics out of the ones that are left. Put the number 4 next to each of those.
Now pick the top 6 of those remaining. Put the number 5 next to them.
There are only 15 topics left. If we could address 5 of those, which would they be? Put the number 6 next to each of those topics.
Pick 4 out of the last 10 topics and put the number 7 next to each of those.
Of the last 6 topics, choose 3 you feel should be worked on right away and put the number 8 next to them.
Now go back to the topics that were your top 3—the ones you marked with the number 1. Pick the one you think should be addressed first, before the other two and change it’s number to 0.
Finally, look at the last 3, the only ones you haven’t marked at all. Pretend we suddenly can work on every single item on the list except for one of these 3 which has to wait until later. Choose that one and put the number 10 next to it. Put the number 9 next to the other two which are still blank.
