and Transportation Services for the Community Integration and Habilitation Waiver

Personal Assistance Care (PAC):
- Providers wanting to add PAC to their list of services must be approved by DDRS to
provide Residential Habilitation and Support (RHS).
- All current DDRS-approved RHS providers wanting to add PAC must submit, in
writing, a request to add this service to BDDS Provider Relations, and BDDS Provider
Relations will add the PAC service. Please email the request to Beth Goodrich at
Beth.Goodrich@fssa.in.gov.
Transportation Services—CIH Waiver only:
- Depending upon the needs of the participant, there are three levels of
transportation. The level of transportation service needed must be documented in
the ISP.
o Level 1: Transportation in a private, commercial, or public transit vehicle that
is not specially equipped.
Rate: $5 per trip
Cap: $2,500
o Level 2: Transportation in a private, commercial, or public transit vehicle
specially designed to accommodate wheelchairs.
Rate: $20 per trip
Cap: $5,000
o Level 3: Transportation in a vehicle specially designed to accommodate a
participant who for medical reasons must remain prone during
transportation (e.g., ambulette).
Rate: $40 per trip
Cap: $7,500
Transportation is to be paid separate from the individual’s current allocation.
- Beginning September 1, providers should request the individual’s case manager
update the CCB to include the appropriate level of transportation.
- RHS and Day Service providers are eligible to provide transportation service on the
CIH Waiver. Current DDRS-approved RHS and Day Service providers wishing to add
Transportation should send a written request to add this service to BDDS Provider
Relations, and BDDS Provider Relations will add the Transportation service. Please
email the request to Beth Goodrich at Beth.Goodrich@fssa.in.gov.
**Note: Transportation has not changed for the FSW.
