Important family supports waiver application information

On Sept 14, Family Voices Indiana shared information about the new application paperwork for the family supports waiver. It included this information:

We (the state) did update our application for services in an attempt to make the application more simple and clear to individuals and families. The Application For Developmental Disability Services can be located on the web at http://www.in.gov/fssa/ddrs/3349.htm.    Currently we are still accepting the old forms until December. 

We did not, however, get this information about additional paperwork that is needed:

HIPAA form (embedded)

Confirmation of Diagnosis, found electronically at www.IN.Gov/fssa/2328.htm  

Families and individuals who wish to apply for the family supports waiver should complete these 3 forms as part of the application process. You can also request that the forms be mailed to you via the BDDS office. You can find a list of those here:

If you believe your child is eligible for the CIH waiver, which is a needs based waiver, you should contact BDDS.

It is also important to contact BDDS the year your child turns 6 to ask about the eligibility process at that time. You should continue to contact BDDS every year to update your contact info or use the waiting list portal:  http://www.in.gov/fssa/ddrs/4328.htm
