Stakeholder survey for FS/CIH waiver renewals

The Division of Disability and Rehabilitative Services will be pursuing renewals of the Community Integration and Habilitation waiver, and the Family Supports waiver. The renewal process is required by the Centers for Medicare and Medicaid Services for the continued operation of these waivers. As part of these renewals, DDRS is reaching out to stakeholders to collect information about service capacity, experiences and access to services on each waiver.

We are encouraging providers, case managers, individuals and families, to complete an online survey. The survey takes approximately 15 minutes to complete. Your responses are anonymous, and your answers will be used to improve and enhance services and supports offered by the Bureau of Developmental Disabilities Services.
