2021 is the year of the reset at Child Neurology Foundation, as we both celebrate our 20th anniversary and look ahead to setting our focus for the next 20 years.
And we want YOU to be part of that!
If you care for a child or youth living with a neurologic condition, as a parent, caregiver or as a neurologist who directly serves children, we want to hear from you about your needs. Sharing your feedback and experience will help us set our priorities and ensure that we best serve you, our community, in the years to come.
And as a small thank you, this year at the end of each short survey (10-15 minutes to complete) you can provide your name and email to be entered in a drawing for a gift card. Better yet, each time someone completes the survey and mentions you as the referral, you're entered again. In short, the more you share, the better your chances of winning a gift card.
