CSHCS & Religious Exemptions

The Indiana Department of Health’s Children’s Special Health Care Services (CSHCS) program provides supplemental medical coverage to help families of children (birth – 21 years of age) who have serious chronic medical conditions. Families need to meet the program’s financial (income no greater than 250% federal poverty level) and medical criteria, in order for the program to help pay for treatment related to their child’s condition. 

The program requires that all applicants make application for Medicaid. However, the program does allow exceptions for religious reasons. The required verification of religious exception is an approved copy of the IRS Form 4029. This form states that the applicant/guardian is exempted from paying Social Security and Medicare taxes due to their religious affiliation. If IRS Form 4029 is not attainable; the CSHCS program will accept a front and back copy of the most recently filed Federal Tax Form 1040, 1040A or 1040EZ. These forms are also acceptable income verification.
