Caregivers' Experiences in the Transitioning from Hospital to the School Setting of Children with Special Health-care Needs
Vanderbilt University is currently seeking survey participants, caregivers of children ages 2-22 who have special health-care needs, especially children with disabilities who also present with special health-care needs. The anticipated outcome will be perspectives and experiences with the transition process from hospital to school. The anonymous, web-based survey should take 10-15 minutes to complete.
