Feedback Wanted - Youth or Young Adults Between the Ages of 12 - 26 with I/DD
The LifeCourse Nexus is working with national partners to create Health Care Transition resources to support youth and young adults with Intellectual and Developmental Disabilities (I/DD). The goal is support youth and young adults with I/DD to learn about leading their own health care as they move into adulthood. The resources include Health Care Transition Kits that each explore a topic. This work is part of the Center for Transition to Adult Health Care for Youth with Disabilities, led by Family Voices.

Participation Involves:
- Attending 8 virtual meetings (approximately 90 minutes each) between July 2024 and April 2025.
- Reviewing a kit in between meetings.
- Providing feedback on each kit during meetings.
- A $100 stipend for each kit reviewed, for up to 6 kits.
