Dear Caregivers,
We wish to invite you to participate in the testing of a new smartphone application, “Caremap”, specifically designed to support caregivers of medically complex children. In appreciation of your time and feedback, you will receive a $100 e-gift card.
At Boston Children’s Hospital and Duke Health, we find that one of the greatest challenges for our families is keeping everyone up to date on the “what happened” and “what’s next” for their child’s care. To support parents and guardians of medically complex children, our teams developed an Apple application called Caremap. With this app, you can:
ü Manage your child’s medication, health problems, allergies, hospitalizations, etc.
ü Track ongoing symptoms such as sleep, bowel movements, or exercise
ü Document and monitor progress towards your child’s health goals
ü Create summaries automatically of your child’s care to share with providers
In order to test the application, we want to invite you to participate in a simulation experience. This test allows participants to explore the app while using a sample patient profile and then provide feedback through a survey. Once you enroll, the entire simulation should take approximately one hour to complete. Of note, the test is only available in English and for iPad and iPhone users at this time.

All gift cards will be sent electronically, within 21 business days from when you submit the survey. Only (1) $100 incentive per participant and per e-mail address.
Your participation is completely voluntary. Further, your child’s healthcare data will not be used or asked about in this “Sim.”
The maximum number of participants for the simulation experience is 50 participants. Enrollment will be based on the first fifty participants to submit their email to the Caremap team through the following link. In the event that we reach out our maximum number of participants and you have submitted your email, we will notify you and put you on a wait list for participation.
Caremap is a finalist in MCHB’s (Maternal & Child Health Bureau) Grand Challenge for solutions to help in care coordination for Children with Special Health Care Needs (CSHCN), The feedback collected through this simulation experience will be aggregated and submitted in the final report to MCHB.
The Caremap App is under development and there are known limitations to its functionality such as the inability to track the care of more than one child at a time.
In order to receive the $100 e-gift card you must review the “Sim” in the Caremap app and complete the feedback survey in its entirety.
