"The Family and Social Services Administration monitors and oversees the administration of the federally required Level of Care assessment and determination and intake counseling for individuals in need of Nursing Facility Level of Care Assessments. Nursing Facility Level of Care assessments are completed for the Health & Wellness, PathWays for Aging, and Traumatic Brain Injury waivers as well as nursing facility admissions.
FSSA is dedicated to enhancing the quality and efficiency of evaluating functional eligibility to access services and supports. The Level of Care Assessment Representative is a process designed to streamline the functional eligibility evaluation to determine the Nursing Facility Level of Care.
Beginning July 1, 2025, the LCAR will provide:
- Preadmission Screening and Resident Review
- LOC assessments and renewals
- LOC determination recommendations and re-determinations
- Intake counseling for the Indiana PathWays for Aging 60+ population
